BINDERHOOPS LAST LOOK COMBINE - WAIVER, RELEASE, AND INDEMNIFICATION AGREEMENT

READ CAREFULLY. BY SUBMITTING THIS REGISTRATION FORM, YOU ARE AGREEING TO A RELEASE OF LIABILITY AND WAIVER OF CERTAIN LEGAL RIGHTS.

1. ACKNOWLEDGMENT OF RISKS
I acknowledge that participation in basketball activities involves inherent risks, including but not limited to: physical contact with other participants, falls, collisions, sprains, strains, fractures, head injuries, and other bodily injuries that may result in serious injury, paralysis, or death. I voluntarily assume all risks associated with participation.

2. WAIVER AND RELEASE OF LIABILITY
In consideration for being permitted to participate in the Binderhoops tryout event, I, for myself and on behalf of the participant (if a minor), hereby WAIVE, RELEASE, and DISCHARGE Steven F. Binder, Binderhoops, and their respective agents, employees, volunteers, and representatives (collectively "Released Parties") from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant while participating in such activity, whether caused by the negligence of the Released Parties or otherwise.

3. INDEMNIFICATION AND HOLD HARMLESS
I agree to INDEMNIFY, DEFEND, and HOLD HARMLESS the Released Parties from any loss, liability, damage, or costs, including court costs and attorney's fees, that may incur due to my or the participant's participation in said activity, whether caused by negligence of Released Parties or otherwise.

4. MEDICAL TREATMENT AUTHORIZATION
I authorize the Released Parties to secure emergency medical treatment for the participant if necessary, and I agree to be financially responsible for any costs incurred as a result of such treatment. I am aware and understand that I should carry my own health insurance for the participant.

5. PARENT/GUARDIAN CONSENT (For Participants Under 18)
I am the parent or legal guardian of the participant registered through this form. I have the legal authority to consent to this agreement on behalf of the minor participant and bind the minor to its terms.

6. GOVERNING LAW
This agreement shall be governed by the laws of the State of New York without regard to conflict of law principles.
